If you are new to this clinic, please a leave a note of primary concerns and book under "New Client"
We will send a health history form link in email that saves you time to be filled out ahead of time.
We will send health screening questions out 24 hrs before (with our reminder email / text) simply click to confirm your appointment and that is also confirming you are clear of any flu/ illness symptoms.
