Wellness Awaits You is a body positive, trauma-informed, consent-based, LGBTQI-affirming, and disability-friendly facility.
You agree to follow our rules/regulations.
We have the right to refuse or discontinue service(s) for any reason, at any time.
We are not responsible for any lost or stolen article(s) or item(s) while on your premises.
We reserve the right to change rules, policies, and pricing at any time. We will do our best to give at least a minimum 30 day notice.
Private areas will not be undraped/uncovered, or have bodywork performed on them under any circumstances.
Booking a Massage with us is simple: Call the office! We can schedule an appointment for you. Keep in mind that In-Home availability is not set-in-stone. It is a first come, first serve basis.
A link will be sent to your email to complete your paperwork. This is all done online and it is required to keep your appointment.
Appointment Reminders/Confirmations will continue during your care in two ways: 1) You'll receive a reminder text 36 hours prior to your appointment, and an email reminder/confirmation 30 hours before your appointment.
Please make sure you have submitted your completed paperwork the day before your new appointment; if at all possible, submit it as soon as you receive it. This allows us to not only assure you the full time of your session but helps us give you the best of care possible by avoiding any possible conflict due to health history.
Make sure to include all information needed for the person the appointment is for; if scheduling for someone other than yourself.
If this is your first (1) appointment, make sure you have submitted all needed paperwork the day prior to your appointment.
Any requests you have in regard to your session, please make sure to have properly informed your Therapist to avoid confusion. This includes: extra pillows, extra blankets, any animals/children in the home during session, street parking, multi-level home, stairs, etc.)
Your scheduled massage appointment consists of time for consulting before and after your Massage session. This is why we ask that we be allowed to arrive earlier than your scheduled appointment time, and have time after the session as well; outside of set-up/break-down.
Emergency Sessions are available. We offer convenience booking for an additional $40. For more information on this, please call the office.
- Failure to allow us additional time prior to your appointment for set-up/consult, will take time away from your scheduled session.
- Upon arrival to the location chosen for your appointment, if you are not there; time will be taken away from your scheduled session. Depending on how long it takes you to arrive, your appointment will have to be rescheduled; incurring our cancellation fee.
- In the case of the therapist running behind schedule, not only will your appointment time be honored but a $25 discount will be appiled to your bill.
All clients are required to keep a credit card on file and sign a Credit Card Authorization Form before any services will be rendered.
This covers the following policies:
Failure to submit all paperwork by ten o' clock (10 PM) the night before your appointment will trigger the system to automatically cancel your appointment; incurring our 24 hour cancellation fee.
We require 24-hour notice to cancel/reschedule appointments. Failure to give 24-hour notice will result in the full-cost of the booked service(s).
We reserve the right to cancel appointments, due to any: illness, personal emergencies, or inclement weather. We know your time is just as valuable, so incase this were to happen, you would receive a discount on your rescheduled session for the inconvenience.
We have a firm stance on No-Showing your appointments. Continuing to No-Show to your appointments will incur the following penalties:
- (2) appointments in (2) months: you will no longer be allowed to schedule service(s) without pre-paying, and will only be allowed to schedule same-day appointment(s).
- (4) appointments in under six (6) months: you will no longer be allowed In-Home Services for at least 8 months after your last No-Show.
It is required to notify the office in writing of any change in credit card or account information on file prior to the next electronic funds transfer (EFT).
We accept payments in the form of card, cash, or check. Be aware that cash is not kept in the office, so make sure to have exact change. For any check that bounces or is declined, an additional $35 fee will be charged. (The $35 fee is not included in the cost of service(s) rendered, as stated, it is an additional charge.)
We do not offer refunds on massage service(s), gift certificate(s), gift card(s), retail merchandise, or stolen/lost item(s).
Gift Certificate(s) are non-transferable, and cannot be redeemed for cash or gratuities.
Any gift certificate(s) or gift card(s) that were purchased during a promotion for a discounted rate, must be used prior to the expiration date of the promotion/discount. Failure to do so will leave you responsible for paying the difference for the scheduled service(s) past the expiration date. (if applicable)
I understand that the massage I receive from Wellness Awaits You - Therapeutic & Holistic Massage Therapy is for the purpose of stress reduction, pain reduction, relief from muscle tension, increasing circulation, or specific reasons stated by a healthcare professional.
I understand that the massage therapist does not diagnose illness or disease, and does not prescribe Medical Treatment or Pharmaceuticals. I understand that massage therapy is not a substitute for Medical Care and that it is recommended I work with my Primary Caregiver (PCP) for any condition I may have.
I understand that the massage therapist will go over with me what areas they will be working on during my scheduled sessions, and I acknowledge that I will give consent at each session.
I have stated all my known conditions and medications, and I will keep Wellness Awaits You updated on any changes.
Release of Medical Records
Following HIPPA Compliance, we must have written permission from you to release any of your records. By agreeing to our policy, you authorize Wellness Awaits You LLC to release your records or other health care information; including chart notes, reports, correspondence, billing statements, and other written information; to the elected parties for the purposes of processing your claims or transferring care.
(Elected Parties: attorneys, healthcare providers, specialists, insurance companies, case managers, etc.)
Contract for Care
I will participate fully as a member of my healthcare team. I will make sound choices regarding my sessions and plan based upon the information provided by Wellness Awaits You LLC.
I will participate in my own self-care programs and adhere to the plan we select. I agree to communicate with my practitioner any time I feel my well-being is being compromised. I expect my practitioner to provide safe and effective treatment to the best of his/her skills and knowledge.
For In-Home availability you must call the office to schedule. In-Home hours are similar to In-Office hours, it is on a first come, first serve basis.
Available In-Home are 60, 90, or 120 Minute sessions. Additional $15 Therapeutic Upgrades can be added to your session. Ask for a list of available Therapeutic Upgrades!
- In-Home sessions were made for the convenience of the client. Therefore, we've created a fee chart for driving distance. Fees are as follows:
- Within 12 miles of our office location - no fee.
- Up to 20 miles from our office location - $.25/mile.
- 20+ miles from our office location - decided on a case-by-case basis.
The privacy of your personal information is important to our office. Please make sure you have read the entire In-Home Policies & Guidelines, as once you submit this Policy; you will be held accountable for any/all rules, policies, or charges incurred.
