If this is your first time into the clinic, we will also email you a copy of our consent/policy forms and online health history form which can be completed online before your appointment! Reminder email or texts are sent with 24 hours notice. Please respect our cancellation policy; 24 hours notice is required to avoid any cancellation fees regardless of circumstance.
